Good Practices

The professional care of people with multimorbidity and dementia is a key issue and challenge throughout Europe. It requires flexibility, expertise and a great deal of empathy, as different clinical pictures and needs must be taken into account simultaneously.

To provide guidance for professionals,  the SACRED project partners collected and mapped local good practices from their regions (Treviso, Italy; Rotterdam, Netherlands; Valencia, Spain; Tenerife, Spain; Syros, Greece; Flanders, Belgium). These include community-based initiatives, innovative care models, and practical tools that are being used in real-world settings.

The more than 20 compiled good practices show diverse approaches of how professional care quality can be improved, self-determination of patients can be promoted, and resources can be used wisely.
